This policy explains how Elevate Employer Benefits Solutions LLC, doing business as Elevate EBS (“Elevate EBS,” “we,” “us”), collects, uses and protects information in the Elevate360 platform at app.elevateebs.com. Our separate website Privacy Policy covers elevateebs.com.
Who uses Elevate360
Elevate360 is used by employers, their employees and dependents, individual and family members, licensed advisors and brokers, and Elevate EBS staff to enroll in and administer our programs, including the Preventative Care Management Plan (PCMP) and Elevate Everyday Health Access (EHA).
What we collect
- Account information: name, email, phone, role, preferred language and sign-in details.
- Employer information: company name, EIN, addresses, contacts, plan selections and signed agreements.
- Employee, member and dependent information: names, contact details, dates of birth, gender, state, employment and payroll details needed for eligibility and enrollment (such as pay and hours), dependents, and program elections.
- Billing information: plan, invoices, payments and payment status. Card and bank details are entered on pages hosted by our payment providers and are not stored in Elevate360; we keep only limited details such as card brand, last four digits and expiry.
- Advisor information: hierarchy, rates, referral codes and commission records.
- Support information: messages you send through chat, email or support tickets.
- Technical information: log data such as IP address, browser, and actions taken, used for security and troubleshooting. We keep a change log of edits to key records.
Elevate360 is not designed to collect medical records or diagnoses. Health and wellness services in our programs are delivered by independent providers, who handle any clinical information under their own privacy practices and applicable law.
How we use information
- To determine eligibility, enroll participants and give them access to program services.
- To bill employers and members and to record payments.
- To calculate and pay advisor and broker commissions.
- To provide customer support and send service messages.
- To secure, maintain and improve the platform, and to meet legal and tax obligations.
We do not sell personal information, and we do not use it for third-party advertising.
QuickBooks Online (Intuit) data
Elevate EBS connects its own QuickBooks Online company to Elevate360 to record commission payouts. Only an authorized Elevate EBS administrator can connect or disconnect it. When connected, Elevate360 reads our QuickBooks account and vendor lists and creates bills (with the payout statement attached) when a payout is approved. QuickBooks data is used only for that purpose and only for Elevate EBS. It is not sold, not shared with other customers, not shown to anyone outside Elevate EBS’s authorized administrators, and not used to train artificial intelligence models. Connection credentials (access tokens) are kept server-side and are never exposed in the browser. The connection can be disconnected at any time, after which Elevate360 no longer accesses QuickBooks.
Who we share information with
We share information only as needed to run the programs and the platform:
- Service providers that host or support Elevate360, including our application hosting platform (Bubble), our billing, CRM, e-signature, chat and help-desk tools (Zoho), payment processing (Stripe), payroll connection services (Finch), and accounting software (Intuit QuickBooks Online).
- Program vendors that deliver the services participants enroll in (for example, telehealth and related service providers), which receive the enrollment information they need to give access.
- The participant’s employer, advisor or broker, limited to what they need to administer the program. Employers do not see which services an employee uses.
- AI support chat: our chat assistants are powered by Anthropic’s Claude. Messages typed into chat are processed to answer them. We do not send QuickBooks data, payment details or enrollment records to the chat assistants.
- Legal and safety: when required by law, or to protect the rights and safety of our users, Elevate EBS or others.
Security
Data is transmitted over encrypted connections (HTTPS). Access inside Elevate360 is controlled by role-based permissions and privacy rules so users see only the records they are allowed to see. Integration keys and tokens are stored server-side. No system is completely secure; if we learn of a breach affecting your information, we will notify you as required by law.
Retention
We keep information for as long as needed to provide the programs, meet contractual, tax and legal requirements, and resolve disputes, then delete or de-identify it.
Your choices
You can review and update much of your information in your account. To ask for access, correction or deletion of your information, contact support@elevateebs.com. Some information must be kept to meet legal obligations or the terms of an employer’s program.
Children
Elevate360 accounts are for adults. Information about minor dependents is provided by the enrolling parent or guardian and used only to give them access to program services.
Changes
We may update this policy. The “Last updated” date shows when it last changed, and we will give notice of material changes through the platform or by email.
