Elevate EBS (“Elevate EBS,” “we,” “us”) provides employer benefits administration services. This policy explains what we collect through this website, why, and what choices you have. It covers elevateebs.com only.
What we collect
Information you give us. If you complete our contact form, we collect your first name, last name, email address and the content of your message. If you use the live chat, we collect whatever you type into it, along with any name or contact details you provide.
Information collected automatically. Like most websites, our server records standard technical data with each visit: IP address, browser type, device type, pages viewed, and the site you arrived from. We also use Google Analytics, which places cookies in your browser to measure how the site is used.
What we do not collect here
We do not collect health information, medical records, Social Security numbers, or payment details through this website. Enrollment in our programs happens on separate platforms, and the health services offered under those programs are delivered by independent third-party vendors who handle any clinical information under their own privacy practices and applicable law.
This website is a marketing and information site. Nothing you submit here should include sensitive personal or medical information.
